Code CR-4
Problem Description
Corrected claim - the claim has been processed and is not a duplicate. The claim has been processed as a replacement claim.
Root Cause
The payer has processed the corrected claim as a replacement claim, confirming it is not a duplicate. The CR (Correction) group code indicates the claim was submitted with frequency code 7 (replacement) and the payer has accepted it as a replacement for the original claim. The original claim is voided, and the replacement claim is adjudicated in its place. This is the expected outcome when a provider submits a corrected claim — the payer replaces the original adjudication with the new one, which may result in a different payment, denial, or adjustment. The provider should reconcile the replacement claim's adjudication against the original.
Resolution Steps
- 1Review the ERA/EOB for the replacement claim to confirm the original claim was voided and the replacement claim was adjudicated with the corrected information.
- 2Reconcile the replacement claim's payment or adjustment against the original claim's adjudication to identify any payment recoupment, additional payment, or change in denial reason.
- 3If the payer recouped the original payment and issued a new payment, post both transactions in the practice management system and verify the net financial impact.
- 4If the replacement claim was denied with a new reason, follow the resolution steps for the new CARC and determine whether a further appeal or additional correction is needed.
- 5If the payer did not void the original claim and processed both, contact the payer to request a reversal of the original claim to prevent duplicate payment issues.
EHR Documentation Note
Claim processed CR-4 — replacement claim adjudicated; reconciled replacement against original, posted recoupment/new payment, and verified net financial impact.
Preventative Action
Always include frequency code 7 and the original claim number on replacement claims, and reconcile replacement adjudications against original payments within 30 days to catch recoupment errors.
