CO-5Denial CodeContractual Obligation

Code CO-5

Problem Description

The procedure code/type of bill is inconsistent with the place of service. Usage: Refer to the 835 Healthcare Policy Identification Segment (loop 2110 Service Payment Information REF), if present.

Root Cause

The procedure code or type of bill billed is inconsistent with the place of service (POS) reported on the claim. Payers maintain lists of which CPT/HCPCS codes are payable in specific POS settings — for example, an inpatient-only procedure billed with POS 11 (office), or an office visit billed with POS 21 (inpatient hospital). The payer's editing logic cross-references the CPT code against the POS and denies when the combination is not permitted per CPT guidelines, Medicare NCD/LCD policies, or payer-specific POS rules referenced in the 835 Healthcare Policy Identification Segment.

Resolution Steps

  1. 1Review the denied line item against the CPT code set and the payer's published POS policy to determine whether the procedure is allowed in the billed POS.
  2. 2Access the medical record to confirm the actual location where the service was rendered and whether the documentation supports the POS billed.
  3. 3If the POS was entered incorrectly, submit a corrected claim with the accurate POS code that matches the service location and the CPT code requirements.
  4. 4If the POS is correct but the CPT code is not payable in that setting, determine whether a different CPT code applies to the service as rendered in that location, or whether the service must be billed with a different POS.
  5. 5If the denial appears erroneous — for example, the CPT code is explicitly allowed in the billed POS per the payer's own policy — file an appeal with the policy citation and the medical record supporting the service location.

EHR Documentation Note

Claim denied CO-5 — procedure/POS inconsistency; verified service location in medical record, corrected POS or CPT code, and resubmitted corrected claim.

Preventative Action

Maintain a payer-specific POS-to-CPT compatibility matrix in the practice management system and validate POS against the CPT code at the point of charge entry before claim submission.
