Code CO-18
Problem Description
Exact duplicate claim/service (Use only with Group Code OA except where state workers' compensation regulations requires CO)
Root Cause
The claim is an exact duplicate of a previously submitted claim for the same date of service, same procedure code, same billed amount, same units, and same performing provider. The payer's duplicate detection logic flags the claim to prevent overpayment. This can occur due to clearinghouse resubmission, manual re-keying, or a practice management system glitch that re-sends claims. Note: CO-18 is used with Group Code OA (or CO where state workers' compensation requires it), distinguishing it from a corrected claim.
Resolution Steps
- 1Search the practice management system and clearinghouse reports for the original claim using the same DOS, CPT, provider, and billed amount to confirm whether the claim was already paid, denied, or is still pending.
- 2If the original claim was paid, write off the duplicate denial — no further action is needed.
- 3If the original claim was denied and this is a corrected resubmission, verify that the corrected claim was submitted with the appropriate frequency code (7 = replacement) and resubmission reference; if not, resubmit as a corrected claim.
- 4If the services were genuinely distinct (same code, same DOS, but separate encounters), append modifier 76 (repeat procedure by same provider) or 77 (repeat by another provider) and resubmit with documentation supporting the separate encounters.
- 5If the duplicate was caused by a clearinghouse or system error, contact the clearinghouse to prevent future duplicate transmissions and document the root cause for your billing team.
EHR Documentation Note
Claim denied CO-18 — exact duplicate; verified original claim status, applied modifier 76/77 if distinct encounters, or wrote off as valid duplicate denial.
Preventative Action
Configure the practice management system to flag duplicate claims before submission and establish a clearinghouse reconciliation process to catch duplicate transmissions within 24 hours.
