Code CO-26
Problem Description
Expenses incurred prior to coverage.
Root Cause
The expenses or services were incurred prior to the patient's coverage effective date. The payer's eligibility system compares the date of service on the claim to the patient's coverage effective date and denies the claim if the service was rendered before the patient was enrolled in the plan. This commonly occurs when a patient receives services during a coverage transition period, when there is a lag between enrollment and coverage activation, or when the coverage effective date in the payer's system is incorrect. The claim is denied as a contractual obligation — the payer is not responsible for services rendered before the patient was covered.
Resolution Steps
- 1Verify the patient's coverage effective date via a 270/271 eligibility transaction or the payer portal and compare it to the date of service on the claim.
- 2If the coverage effective date in the payer's system is incorrect (e.g., the patient enrolled before the date of service but the system shows a later date), contact the payer's provider relations or member services to correct the enrollment record and resubmit the claim.
- 3If the coverage was genuinely not active on the date of service, determine whether the patient had prior coverage from another payer and submit the claim to that payer.
- 4If the patient had no coverage on the date of service, bill the patient directly or offer financial assistance/charity care options.
- 5If the patient disputes the coverage effective date, obtain a coverage letter from the payer confirming the correct enrollment date and file a claim reconsideration with the documentation.
EHR Documentation Note
Claim denied CO-26 — expenses incurred prior to coverage; verified coverage effective date, corrected enrollment if applicable, or billed patient/billed prior payer.
Preventative Action
Verify coverage effective dates during pre-visit eligibility checks and flag services rendered before the effective date for patient financial counseling before claim submission.
