Code CR-3
Problem Description
Corrected claim - the claim has been processed and is not a duplicate. The claim has been forwarded to another payer.
Root Cause
The payer has processed the corrected claim and determined that it should be forwarded to another payer for adjudication. The CR (Correction) group code confirms this is a corrected claim, not a duplicate, but the payer has identified that another insurer is responsible for payment — this may be due to coordination of benefits, a change in the patient's primary payer, or the corrected claim revealing that the service falls under a different payer's coverage. The claim is being redirected rather than denied, and the provider should expect adjudication from the receiving payer.
Resolution Steps
- 1Verify the patient's current insurance coverage and coordination of benefits order via a 270/271 eligibility transaction to confirm which payer should be primary.
- 2Determine whether the corrected claim revealed information (e.g., a different diagnosis, date, or service) that changed the payer responsibility.
- 3Submit the corrected claim to the correct primary payer with all supporting documentation and the original claim reference.
- 4If the original payer forwarded the claim, follow up with the receiving payer within 14–21 days to confirm they received the forwarded claim and are processing it.
- 5If the receiving payer denies or does not acknowledge the forwarded claim, resubmit directly with a cover letter explaining the payer redirect and include the original payer's EOB showing the forward.
EHR Documentation Note
Claim processed CR-3 — corrected claim forwarded to another payer; verified COB order, submitted to correct payer, and followed up with receiving payer for adjudication.
Preventative Action
Verify coordination of benefits and primary payer responsibility before submitting corrected claims to prevent payer redirects and processing delays.
