CR-2Denial CodeContractual Obligation

Code CR-2

Problem Description

Corrected claim - the claim has been processed and is not a duplicate. Additional information or documentation is required.

Root Cause

The payer has processed the corrected claim but requires additional information or documentation to complete adjudication. The CR (Correction) group code confirms this is a corrected claim, not a duplicate, but the accompanying RARC will specify what additional documentation is needed — this may include operative notes, medical records, itemized statements, or prior authorization documentation. The claim is pended pending receipt of the required documentation. This differs from CR-1 in that the corrected claim was accepted but cannot be fully adjudicated without the additional information.

Resolution Steps

  1. 1Locate the accompanying Remittance Advice Remark Code (RARC) on the ERA/EOB to identify the specific additional information or documentation required.
  2. 2Retrieve the required documentation from the EHR, practice management system, or prior payer records, ensuring it covers the date of service and the corrected claim details.
  3. 3Submit the additional documentation electronically via the payer's provider portal, the clearinghouse attachment service (275 transaction), or by fax/mail with the claim control number or DCN for matching.
  4. 4Track the documentation submission and follow up with the payer within 7–10 business days to confirm receipt and claim reprocessing.
  5. 5If the payer denies the corrected claim after receiving the documentation, review the new denial reason and determine whether a further appeal or additional corrected claim is needed.

EHR Documentation Note

Claim processed CR-2 — corrected claim accepted, additional documentation required; identified required docs per RARC, submitted via payer portal, and tracked for reprocessing.

Preventative Action

Include all relevant documentation with the initial corrected claim submission and review the payer's documentation requirements before resubmission to prevent pends.
