CO-58Denial CodeContractual Obligation

Code CO-58

Problem Description

Treatment was deemed by the payer to have been rendered in an inappropriate or invalid place of service. Usage: Refer to the 835 Healthcare Policy Identification Segment (loop 2110 Service Payment Information REF), if present.

Root Cause

The payer determined that the treatment was rendered in an inappropriate or invalid place of service. Unlike CO-5 (where the CPT code is incompatible with the POS), CO-58 focuses on the clinical appropriateness of the treatment setting — for example, a service that should have been performed in an outpatient hospital was performed in an office setting, or a service requiring 24-hour monitoring was performed in an ambulatory setting. The payer applies clinical policies and coverage guidelines to determine the appropriate setting for each procedure and denies claims where the service was rendered in a setting that does not meet the clinical requirements for safe and effective care.

Resolution Steps

  1. 1Review the payer's coverage policy for the denied CPT code to determine the clinically appropriate place of service settings the payer allows.
  2. 2Access the medical record to confirm the actual setting where the service was rendered and whether the documentation supports the medical necessity of the setting.
  3. 3If the POS was entered incorrectly, submit a corrected claim with the accurate POS code that matches the service location and the payer's allowed settings.
  4. 4If the service was genuinely rendered in a non-standard setting but was medically necessary (e.g., emergent circumstances required an alternative setting), file an appeal with clinical documentation justifying the setting choice.
  5. 5If the payer's policy does not allow the billed setting under any circumstances, determine whether the service can be rebilled with a different POS or whether the denial is valid and the balance should be written off.

EHR Documentation Note

Claim denied CO-58 — treatment in inappropriate place of service; verified payer POS policy, corrected POS if applicable, or appealed with clinical justification for setting.

Preventative Action

Maintain a payer-specific list of approved place-of-service settings for high-cost or setting-sensitive procedures and verify the POS against the payer policy before claim submission.
