Code CO-197
Problem Description
Precertification/authorization/notification/pre-treatment absent.
Root Cause
The claim was denied because precertification, authorization, notification, or pre-treatment was absent. The payer requires prior authorization for certain services (e.g., advanced imaging, surgeries, specialty drugs, durable medical equipment) and the claim was submitted without evidence that authorization was obtained. This is one of the most preventable denials — it occurs when the front office or clinical staff fails to initiate the authorization process before the service is rendered, or the authorization was obtained but not linked to the claim.
Resolution Steps
- 1Check the practice management system and payer portal to determine whether a prior authorization was obtained for this service and date of service; if it was, verify the authorization number was entered on the claim.
- 2If no authorization was obtained, contact the payer immediately to request retro-authorization; some payers allow retro-authorization within a limited window (typically 14–30 days) if the service was medically necessary.
- 3If retro-authorization is denied, file a formal appeal with a letter of medical necessity, the clinical documentation supporting the service, and an explanation of the administrative error that caused the missing authorization.
- 4If the authorization was obtained but not linked to the claim, resubmit the corrected claim with the authorization number in the appropriate 837 loop (2300 REF*G1 for institutional, 2300/2400 REF*G1 for professional).
- 5If the payer will not overturn the denial, write off the balance as a provider write-off (if in-network) or bill the patient (if out-of-network and permitted by the plan).
EHR Documentation Note
Claim denied CO-197 — prior authorization absent; verified authorization status, requested retro-auth if applicable, or appealed with medical necessity documentation.
Preventative Action
Maintain a payer-specific prior authorization list in the practice management system with automated alerts for services requiring authorization, and require front-office staff to confirm authorization before scheduling any procedure.
