CO-185Denial CodeContractual Obligation

Code CO-185

Problem Description

The rendering provider is not eligible to perform the service billed. Usage: Refer to the 835 Healthcare Policy Identification Segment (loop 2110 Service Payment Information REF), if present.

Root Cause

The rendering provider is not eligible to perform the service billed. This denial occurs when the rendering provider's credentialing, enrollment, or license type does not qualify them to perform or bill the specific CPT/HCPCS code. Common examples include a provider whose license type does not cover the service (e.g., a licensed professional counselor billing a medical procedure), a provider whose credentialing with the payer has not been completed or has expired, or a provider whose NPI is not enrolled for the specific service category. The payer cross-references the rendering provider NPI against its enrollment database and denies claims from providers not eligible for the billed service.

Resolution Steps

  1. 1Verify the rendering provider's NPI, license type, and scope of practice against the CPT/HCPCS code billed to confirm the provider is eligible to perform the service.
  2. 2Check the provider's credentialing and enrollment status with the payer to confirm the provider is enrolled and active for the specific service type and date of service.
  3. 3If the wrong rendering provider NPI was entered on the claim, correct the provider information and resubmit the claim with the appropriate rendering provider.
  4. 4If the provider's credentialing is pending or expired, contact the payer's credentialing department to expedite the enrollment and resubmit the claim once the credentialing is active.
  5. 5If the provider's license type genuinely does not cover the service, write off the balance or determine whether the service can be rebilled under a supervising provider if the payer's rules allow.
