Code CO-15
Problem Description
Authorization absent or incomplete for the service billed.
Root Cause
The claim was denied because the required prior authorization or notification for the service was absent or incomplete. This is similar to CO-197 but may be used by payers to indicate a broader authorization deficiency — the service may have required a notification (not a full authorization), the authorization was obtained but expired before the date of service, or the authorization number was present but did not match the service billed. Payers maintain lists of services requiring prior authorization and deny claims that do not have a matching, active authorization on file for the specific date of service and procedure code.
Resolution Steps
- 1Check the practice management system and payer portal to determine whether a prior authorization or notification was obtained for this service and date of service.
- 2If an authorization was obtained, verify the authorization number, date range, and covered CPT codes match what was billed on the claim.
- 3If no authorization was obtained, contact the payer to request retro-authorization within the allowed window, providing clinical documentation supporting medical necessity.
- 4If retro-authorization is denied, file a formal appeal with a letter of medical necessity, the clinical documentation, and an explanation of the administrative oversight.
- 5If the authorization was expired or did not cover the specific service, obtain a new authorization if possible or appeal with documentation justifying the service beyond the original authorization scope.
EHR Documentation Note
Claim denied CO-15 — authorization absent or incomplete; verified authorization status, requested retro-auth or appealed with medical necessity documentation.
Preventative Action
Maintain a payer-specific prior authorization list with automated expiration alerts and require front-office staff to verify authorization validity for the specific DOS and CPT code before service delivery.
