PI-104Adjustment CodePatient Responsibility

Code PI-104

Problem Description

Managed care withholding.

Root Cause

The payer has withheld a portion of the payment as a managed care withholding. Managed care organizations (MCOs) and certain payer contracts include provisions that allow the payer to withhold a percentage of each claim payment as a financial incentive for providers to meet quality, utilization, or performance metrics. The withheld amount is typically held in a reserve account and may be released to the provider at the end of the contract year if performance targets are met. This is a patient responsibility adjustment (PI group) in some plan structures, meaning the withheld amount may be billed to the patient or written off depending on the provider's contract terms. The withholding is not a denial but a deferred payment mechanism.

Resolution Steps

  1. 1Review the provider's managed care contract to understand the withholding percentage, the performance metrics, and the schedule for releasing withheld funds.
  2. 2Verify the withholding amount on the ERA/EOB against the contract's specified withholding rate and the allowed amount for the claim.
  3. 3Post the withholding adjustment in the practice management system as a deferred receivable, not a write-off, since the funds may be recovered at year-end.
  4. 4If the withholding was applied in error or exceeds the contractually specified rate, file a payment dispute with the payer citing the contract terms and the incorrect withholding calculation.
  5. 5At contract year-end, reconcile all withheld amounts against the payer's performance reconciliation and pursue recovery of any released funds.
