Code PR-3
Problem Description
Co-payment Amount
Root Cause
The co-payment (copay) amount has been applied to this claim as a patient responsibility adjustment. After adjudication, the payer determined that the patient owes a fixed copay amount for the service rendered, as defined by their insurance plan. Copays are most common in HMO and PPO plans for office visits, specialist visits, urgent care, and prescription drugs. The adjustment is informational — the claim is not denied, but the patient balance must be collected. The copay amount is typically a flat dollar amount (e.g., $20, $40) rather than a percentage.
Resolution Steps
- 1Verify the patient's copay amount from the eligibility/benefits response (270/271 transaction) or the payer's member portal to confirm the applied amount matches the plan's copay schedule for the specific service type.
- 2Cross-check the copay against the allowed amount on the ERA to ensure the payer applied the correct fixed amount and did not exceed the plan's copay limit.
- 3Bill the patient for the copay amount with a clear statement showing the payer's payment, the allowed amount, and the patient responsibility portion.
- 4If the patient has secondary insurance that covers copays, submit the claim to the secondary payer with the primary EOB attached for coordination of benefits.
- 5If the patient disputes the copay or the amount appears incorrect, request a claim reconsideration from the payer with the benefits verification documentation.
