Code OA-18

Problem Description

Exact duplicate claim/service (Use only with Group Code OA)

Root Cause

The claim is an exact duplicate of a previously submitted claim, identified with Group Code OA (Other Adjustment) rather than CO. The payer's duplicate detection system flagged the claim because it matches an existing claim for the same date of service, procedure code, billed amount, units, and performing provider. OA-18 is used when the duplicate is not a contractual obligation but rather an administrative duplicate — this can result from clearinghouse retransmissions, practice management system glitches, or manual re-keying. The key distinction is that OA indicates the adjustment reason is neither a contractual obligation (CO) nor patient responsibility (PR).

Resolution Steps

  1. 1Search the practice management system and clearinghouse reports for the original claim using the same DOS, CPT, provider, and billed amount to determine whether the original was paid, denied, or is still pending.
  2. 2If the original claim was paid, write off the duplicate denial as a valid adjustment — no further action is needed.
  3. 3If the original claim was denied and this is a corrected resubmission, verify the corrected claim was submitted with frequency code 7 (replacement) and the original claim reference; if not, resubmit as a corrected claim.
  4. 4If the services were genuinely distinct (same code, same DOS, separate encounters), append modifier 76 (repeat procedure by same provider) or 77 (repeat by another provider) and resubmit with documentation supporting the separate encounters.
  5. 5If the duplicate was caused by a clearinghouse or system error, contact the clearinghouse to prevent future duplicate transmissions and document the root cause for process improvement.

EHR Documentation Note

Claim denied OA-18 — exact duplicate (Other Adjustment); verified original claim status, applied modifier 76/77 if distinct encounters, or wrote off as valid duplicate.

Preventative Action

Configure the practice management system to flag duplicate claims before submission and establish a clearinghouse reconciliation process to catch duplicate transmissions within 24 hours.
