Code 59

Problem Description

Processed based on multiple or concurrent procedure rules. (For example multiple surgery or diagnostic imaging, concurrent anesthesia.) Usage: Refer to the 835 Healthcare Policy Identification Segment (loop 2110 Service Payment Information REF), if present.

Root Cause

The claim was processed based on multiple or concurrent procedure rules. This is an adjustment, not a denial — the payer applied bundling or multiple-procedure payment rules to the claim. Common scenarios include multiple surgery rules (where the most complex procedure is paid at 100% and subsequent procedures are paid at 50%), concurrent anesthesia rules, or diagnostic imaging payment rules. The payer's adjudication system applies National Correct Coding Initiative (NCCI) edits or payer-specific multiple-procedure logic to reduce payment for secondary and subsequent procedures on the same claim or the same date of service.

Resolution Steps

  1. 1Review the ERA/EOB to understand which multiple-procedure rule was applied (multiple surgery, concurrent anesthesia, diagnostic imaging) and which procedures were affected by the payment reduction.
  2. 2Verify that the procedures were correctly bundled per CPT guidelines, NCCI edits, or the payer's multiple-procedure policy; if the bundling is correct, post the adjusted payment.
  3. 3If the procedures were distinct and separately reportable (e.g., separate anatomic sites, separate encounters, or distinct services), append the appropriate modifier (59, XE, XS, XP, XU) and submit a corrected claim or appeal.
  4. 4Cross-check the multiple-procedure reduction calculation against the payer's published payment rules to ensure the correct percentage was applied to the correct procedures.
  5. 5If the reduction was applied in error — for example, the procedures were on different dates of service or different anatomic sites — file a payment dispute with the supporting documentation.

EHR Documentation Note

Claim adjusted 59 — multiple/concurrent procedure rules applied; verified bundling per NCCI/CPT guidelines, posted adjusted payment, or appealed with modifier if services were distinct.

Preventative Action

Run NCCI and multiple-procedure edit checks in the practice management system before claim submission and train coders on multiple surgery, concurrent anesthesia, and diagnostic imaging payment rules.
