Code 35

Problem Description

Lifetime benefit maximum has been reached.

Root Cause

The patient has reached the lifetime benefit maximum under their insurance plan. Some plans impose a lifetime cap on certain benefits — most commonly mental health, substance abuse, rehabilitation, or transplant services — after which no further benefits are payable for that category. The payer tracks accumulated payments toward the lifetime maximum and denies claims once the cap is reached. Note that the Affordable Care Act (ACA) eliminated lifetime limits on essential health benefits for most plans, but grandfathered plans, certain non-essential benefits, and self-funded plans may still have lifetime caps in effect.

Resolution Steps

  1. 1Verify the patient's lifetime benefit maximum status by contacting the payer's provider relations or checking the payer portal for the accumulated benefit usage and the remaining balance.
  2. 2Confirm whether the lifetime maximum applies to the specific service billed and whether the plan is subject to ACA essential health benefit requirements that prohibit lifetime limits.
  3. 3If the lifetime maximum has been reached and the plan is ACA-compliant for essential health benefits, file an appeal citing the ACA prohibition on lifetime limits for essential health benefits.
  4. 4If the lifetime maximum is valid (e.g., non-essential benefit, grandfathered plan), determine whether the patient has secondary insurance that may cover the service and submit the claim with the primary EOB.
  5. 5If no other coverage is available, bill the patient directly or offer financial assistance/charity care options for the non-covered service.

EHR Documentation Note

Claim denied 35 — lifetime benefit maximum reached; verified benefit cap status, filed appeal if ACA-protected, or billed patient/secondary payer.

Preventative Action

Verify lifetime benefit maximum status during pre-visit eligibility checks for services commonly subject to caps and inform patients of remaining benefits before service delivery.
