Code 204

Problem Description

This service/equipment/drug is not covered under the patient's current benefit plan

Root Cause

The service, equipment, or drug is not covered under the patient's current benefit plan. This is a plan-level exclusion — the service may be covered under a different plan tier or a different payer, but the patient's current plan does not include it as a benefit. Common examples include cosmetic procedures, fertility treatments, experimental drugs, or durable medical equipment not included in the plan's formulary or benefit schedule. Unlike CO-50 (medical necessity), this denial means the service is excluded from the plan entirely.

Resolution Steps

  1. 1Review the patient's benefit plan documents or summary plan description (SPD) to confirm whether the service, equipment, or drug is an excluded benefit under the current plan.
  2. 2Contact the payer's provider relations or member services to obtain a written explanation of the benefit exclusion and confirm there is no pathway for coverage (e.g., exception process, formulary override).
  3. 3If the plan has a formulary exception or coverage exception process, submit a request with supporting clinical documentation and a letter of medical necessity from the treating provider.
  4. 4If the service is genuinely excluded, determine whether the patient signed an Advance Beneficiary Notice (ABN) or financial responsibility form; if so, bill the patient for the non-covered amount.
  5. 5If the patient has secondary insurance that may cover the service, submit the claim to the secondary payer with the primary EOB showing the non-covered determination.

EHR Documentation Note

Claim denied 204 — service not covered under benefit plan; verified plan exclusions, submitted formulary exception if applicable, or billed patient with ABN.

Preventative Action

Verify benefit coverage for non-standard services during pre-visit eligibility checks, obtain ABNs for services likely to be excluded, and maintain a list of commonly excluded services by payer.
