Code 189

Problem Description

'Not otherwise classified' or 'unlisted' procedure code (CPT/HCPCS) was billed when there is a specific procedure code for this procedure/service

Root Cause

The claim was denied because a "not otherwise classified" (NOC) or "unlisted" procedure code was billed when a more specific code was available. NOC and unlisted codes (e.g., CPT 99199 unlisted special medicine service, 64999 unlisted nervous system procedure) are intended for services that do not have a specific CPT code. The payer's editing system or medical review staff determined that a specific CPT code exists for the service rendered and denied the unlisted code. This denial indicates the provider should have used the specific code rather than the unlisted code, and the payer requires the claim to be resubmitted with the correct specific CPT code.

Resolution Steps

  1. 1Review the medical record to determine the specific service rendered and identify whether a specific CPT/HCPCS code accurately describes the service performed.
  2. 2If a specific CPT code exists for the service, correct the claim by replacing the unlisted code with the specific code and resubmit the corrected claim.
  3. 3If no specific CPT code exists and the unlisted code is the only appropriate code, file an appeal with a detailed description of the service, a coding rationale letter, and comparison to similar codes to justify the use of the unlisted code.
  4. 4Include the operative note, office note, or procedure documentation that describes the service in sufficient detail to support the unlisted code billing.
  5. 5If the payer continues to deny the unlisted code, request a manual review or escalate the appeal to a higher level with a certified coder's rationale supporting the use of the unlisted code.

EHR Documentation Note

Claim denied 189 — unlisted code billed when specific code available; reviewed medical record, corrected to specific CPT code, or appealed with coding rationale if unlisted code was appropriate.

Preventative Action

Train coders to identify specific CPT codes before defaulting to unlisted codes and implement a pre-submission coding review for all claims using NOC or unlisted procedure codes.
