Code 182

Problem Description

Procedure modifier was invalid on the date of service.

Root Cause

The procedure modifier reported on the claim was invalid on the date of service. This occurs when a modifier was deleted, revised, or not yet effective on the date of service — for example, using a modifier that was discontinued in a prior CPT update, or using a new modifier that was not yet effective on the DOS. The payer's editing system validates each modifier against the CPT/HCPCS modifier effective dates and denies claims where the modifier was not valid on the service date. This can also occur when a modifier is used with a CPT code that does not permit that modifier.

Resolution Steps

  1. 1Verify the effective date of the modifier on the claim against the current CPT/HCPCS modifier list and the date of service to determine whether the modifier was valid on that date.
  2. 2Review the medical record and the CPT code to confirm whether a different, valid modifier should be used or whether no modifier is needed.
  3. 3If the modifier was not valid for the DOS, correct the modifier to one that was effective on the date of service and resubmit the claim, or remove the modifier if it is not required.
  4. 4If the modifier was valid for the DOS but the payer's editing system incorrectly flagged it, file an appeal with the CPT/HCPCS modifier documentation showing the effective date.
  5. 5Update the EHR and practice management system with the current CPT/HCPCS modifier set to prevent future use of expired or not-yet-effective modifiers.

EHR Documentation Note

Claim denied 182 — invalid modifier for DOS; verified modifier effective date, corrected or removed modifier, and resubmitted claim.

Preventative Action

Update the EHR and practice management system with the latest CPT/HCPCS modifier set annually and validate modifier effective dates against the DOS at the point of charge entry.
