Code 172

Problem Description

Payment is adjusted when performed/billed by a provider of this specialty. Usage: Refer to the 835 Healthcare Policy Identification Segment (loop 2110 Service Payment Information REF), if present.

Root Cause

Payment is adjusted because the service was performed or billed by a provider of a specialty that triggers a payment reduction or adjustment under the payer's rules. Unlike CO-170 (where the provider type is denied entirely), CO-172 is an adjustment — the service is covered but the payment is reduced because the provider's specialty does not qualify for the full payment rate. For example, a service performed by a non-physician provider (NP, PA) may be paid at a reduced rate (e.g., 85% of the physician fee schedule for Medicare), or a service performed by a specialist in a non-preferred specialty category may receive a reduced reimbursement.

Resolution Steps

  1. 1Review the ERA/EOB to understand the payment adjustment and confirm the reduction is based on the provider's specialty classification.
  2. 2Verify the rendering provider's taxonomy code and specialty designation in the payer's enrollment records to ensure the correct specialty is on file.
  3. 3If the provider's specialty was incorrectly classified in the payer's system, contact provider relations to correct the enrollment record and file a claim reconsideration for the adjusted payment.
  4. 4If the adjustment is correct per the payer's payment rules (e.g., NP/PA services paid at 85%), post the adjusted payment and verify the patient is not balance-billed for the reduction if the provider is in-network.
  5. 5If the adjustment appears to exceed the standard reduction rate or was applied in error, file a payment dispute with the payer citing the contract terms and the expected payment rate.

EHR Documentation Note

Claim adjusted 172 — payment adjusted for provider specialty; verified provider specialty enrollment, posted adjusted payment, or filed dispute for incorrect specialty classification.

Preventative Action

Verify provider specialty enrollment status with each payer and ensure the correct taxonomy code is on file to prevent incorrect specialty-based payment adjustments.
