Code 151

Problem Description

Payment adjusted because the payer deems the information submitted does not support this many/frequency of services.

Root Cause

The payer adjusted the payment because the information submitted on the claim does not support the level of service billed. This commonly occurs with Evaluation and Management (E&M) codes where the documentation does not support the level billed (e.g., billing a 99214 but the documentation supports only a 99213), or with procedures where the documentation does not support the complexity or intensity reported. The payer may have performed a pre-payment or post-payment medical review and downcoded the service.

Resolution Steps

  1. 1Obtain the payer's review findings or the remittance advice remark code that explains why the submitted information does not support the service level billed.
  2. 2Review the medical record against the documentation guidelines for the billed code (e.g., 1995/1997 E&M guidelines, MDM table, or CPT code-specific documentation requirements) to assess whether the documentation supports the billed level.
  3. 3If the documentation supports the original level, file an appeal with the complete medical record, a coding rationale letter, and references to the applicable documentation guidelines.
  4. 4If the documentation does not support the original level, accept the payer's adjustment and post the reduced payment; educate the provider on documentation requirements to prevent future downcoding.
  5. 5If the payer applied a penalty or recoupment in addition to the adjustment, request a detailed audit report and determine whether a formal appeal or a voluntary refund is appropriate.

EHR Documentation Note

Claim adjusted 151 — information does not support service level; reviewed documentation against E&M/CPT guidelines, appealed if supported, or accepted downcode and educated provider.

Preventative Action

Conduct quarterly internal E&M coding audits using certified coders, provide documentation feedback to providers, and implement EHR-based coding calculators that suggest the appropriate level based on documented MDM.
