Code 150
Problem Description
Payer deems the information submitted does not support this level of service.
Root Cause
The payer deems the information submitted on the claim does not support the level of service billed. This is similar to CO-151 but is used as a denial rather than an adjustment — the payer has reviewed the documentation (either pre-payment or post-payment) and determined that the billed E&M level or procedure code is not supported by the documentation. Common examples include billing a 99214 (level 4 established patient) when the documentation supports only a 99213 (level 3), or billing a comprehensive procedure code when the documentation supports only a limited code. The payer may downcode the service or deny it entirely.
Resolution Steps
- 1Obtain the payer's review findings or the RARC that explains why the submitted documentation does not support the billed level of service.
- 2Review the medical record against the documentation guidelines for the billed code (1995/1997 E&M guidelines, MDM table, or CPT code-specific documentation requirements) to assess whether the documentation supports the billed level.
- 3If the documentation supports the original level, file a formal appeal with the complete medical record, a coding rationale letter, and references to the applicable documentation guidelines.
- 4If the documentation does not support the original level, accept the payer's determination, post the adjusted payment if one was made, and educate the provider on documentation requirements to prevent future downcoding.
- 5If the payer denied the claim entirely rather than downcoding it, determine whether a corrected claim with a lower-level code is appropriate or whether the appeal should proceed with the original code.
EHR Documentation Note
Claim denied 150 — documentation does not support service level; reviewed against E&M/CPT guidelines, appealed if supported, or accepted downcode and educated provider.
Preventative Action
Conduct quarterly internal E&M coding audits using certified coders, provide documentation feedback to providers, and implement EHR-based coding calculators that suggest the appropriate level based on documented MDM.
