Code 119

Problem Description

Benefit maximum for this time period or occurrence has been reached.

Root Cause

The benefit maximum for this time period or occurrence has been reached. Unlike a lifetime maximum (CO-35), this is a time-limited or occurrence-limited cap — for example, a plan may limit physical therapy visits to 20 per year, chiropractic visits to 12 per year, or ambulance trips to a certain number per quarter. Once the limit is reached for the specified time period, subsequent claims are denied until the period resets. The payer tracks accumulated usage and denies claims that exceed the benefit limit. This is a plan benefit limitation, not a medical necessity denial.

Resolution Steps

  1. 1Verify the patient's benefit maximum for the specific service category and time period by contacting the payer or checking the payer portal for accumulated usage and the remaining balance.
  2. 2Confirm the benefit limit applies to the specific CPT/HCPCS code billed and that the accumulated usage count is accurate.
  3. 3If the benefit maximum has been reached and the limit is valid, inform the patient of the benefit limitation and determine whether the service can be billed to the patient directly with a signed ABN.
  4. 4If the patient has secondary insurance that covers services beyond the primary benefit limit, submit the claim to the secondary payer with the primary EOB showing the benefit maximum denial.
  5. 5If the benefit limit appears incorrect — for example, the payer's usage count includes claims that were denied for other reasons — file a claim reconsideration with documentation of the actual covered service count.

EHR Documentation Note

Claim denied 119 — benefit maximum for time period reached; verified benefit limit and usage count, billed patient with ABN, or submitted to secondary payer with primary EOB.

Preventative Action

Track benefit-limited service usage (PT, chiropractic, ambulance, etc.) in the practice management system and inform patients when they are approaching their benefit cap before scheduling additional visits.
