Code 109

Problem Description

Claim/service not covered by this payer/contractor. You must send the claim/service to the correct payer/contractor.

Root Cause

The claim or service is not covered by this payer or contractor. The patient must send the claim to the correct payer. This occurs when a claim is submitted to the wrong insurance company — for example, sending a Medicare Advantage claim to Traditional Medicare, sending a commercial claim to a payer that does not insure the patient, or submitting a claim to a Medicare Administrative Contractor (MAC) that does not serve the patient's geographic region. The denial is administrative, not clinical — the claim simply went to the wrong payer.

Resolution Steps

  1. 1Verify the patient's active insurance coverage via a 270/271 eligibility transaction or the payer portal to identify the correct primary payer and its correct billing address or electronic payer ID.
  2. 2Determine whether the claim was sent to the wrong payer due to a wrong insurance card on file, an outdated payer ID in the practice management system, or a Medicare Advantage plan misidentified as Traditional Medicare.
  3. 3Resubmit the claim to the correct payer with the correct member ID, group number, and electronic payer ID; if submitting electronically, verify the clearinghouse routing.
  4. 4If the claim was correctly submitted but the payer denied it in error, file an appeal with proof of eligibility (271 response or portal screenshot) showing the patient is covered by this payer.
  5. 5Update the patient's insurance record in the practice management system with the correct payer information to prevent future misdirected claims.
