Code 10

Problem Description

The diagnosis is inconsistent with the patient's gender. Usage: Refer to the 835 Healthcare Policy Identification Segment (loop 2110 Service Payment Information REF), if present.

Root Cause

The diagnosis code reported on the claim is inconsistent with the patient's gender. The payer's editing logic cross-references the ICD-10-CM diagnosis code against the patient's gender on file and denies claims where the diagnosis is clinically impossible for the reported gender. Common examples include billing a prostate-related diagnosis (e.g., N40.0 benign prostatic hyperplasia) for a female patient, or an ovarian diagnosis (e.g., N83.0 ovarian cyst) for a male patient. This typically results from a data entry error, an incorrect patient gender on the demographic record, or a wrong diagnosis selected from the problem list.

Resolution Steps

  1. 1Verify the patient's gender in the practice management system against the insurance card and a government-issued ID to confirm the gender on the claim is correct.
  2. 2Review the medical record to confirm the actual diagnosis documented for the date of service and compare it against the ICD-10-CM code on the claim.
  3. 3If the diagnosis code was entered incorrectly or the wrong diagnosis was selected from the problem list, correct the ICD-10-CM code to one that is clinically appropriate for the patient's gender and the documented condition.
  4. 4If the patient's gender was incorrectly recorded in the demographic record, update the patient's gender in the practice management system and resubmit the corrected claim.
  5. 5If the diagnosis is clinically valid but the payer's editing logic flagged it in error (e.g., gender-specific diagnosis in a transgender patient), file an appeal with the medical record and a letter explaining the clinical context.

EHR Documentation Note

Claim denied CO-10 — diagnosis/gender inconsistency; verified patient gender, corrected ICD-10-CM code or demographic record, and resubmitted corrected claim.

Preventative Action

Configure the EHR to validate gender-specific ICD-10-CM diagnosis codes against the patient's gender at the point of charge entry and flag mismatches for coder review before claim release.
